PHM in a Nutshell
When the value of analyzing data to improve population health was first identified back in the 50’s no one knew how big this could potentially get. Suddenly, the true potential of this cosmic shift in healthcare began to crystallize with the advent of high levels of adoption rates in hospitals implementing EMR and EHRs, aided by the 2009 HITECH act.
PHM allows organizations to identify critical categories of patients and, by adopting preventative measures, improve the “at risk” group’s health. In doing so, this reduces the overall cost of healthcare for the patient and depending on the government program at the time (currently the ACA), hospitals can share in the savings achieved by these improvements. Hence, rewards can be reaped for improved population health.
The fundamental shift comes in moving from a Fee for Service (FFS) methodology that incentivizes hospitals to provide services (not necessarily in the patient’s best interest). To a new paradigm of Fee for Value (FFV) where the money spent is measured against the overall health of the target population. The premise is simple. The successful application of this program is less than easy.
